Holistic assessment

A joined-up assessment that brings together medical issues, medications, memory and thinking, day-to-day function, support needs and what matters most to the person.

Frailty and function

Assessment of frailty, reduced resilience, falls risk, weight loss and functional change, with attention to how these affect daily life. Working with the patient, Dr Wright develops an agreed plan aimed at improving or maintaining independence and living well in a place of their choosing.

Memory and cognition

Assessment of memory concerns, confusion and changes in thinking — including whether symptoms may relate to delirium, a dementia subtype or another cause. Where relevant, Dr Wright assesses non-financial capacity and works with the patient and family to understand what is happening and develop a kind, practical plan for what comes next.

Complex multimorbidity

Review of several complex medical conditions and how they interact, including the impact on daily life and associated medications, with rationalisation where appropriate. Where relevant, she also supports patients and families living with advanced disease, including priorities, symptom burden and future planning.

Falls and syncope

Assessment of recurrent falls, balance problems and unexplained blackouts or collapses in older adults. Dr Wright works with the patient to get to the bottom of what is causing the problem — and agrees a practical plan, whether that means further investigation, changes to treatment, or steps to reduce risk and build confidence.

Care planning

Clear, practical recommendations for current and future health and care needs — with a focus on supporting independence and avoiding unnecessary hospital admissions. This includes agreed next steps, follow-up, further investigations where appropriate, and coordination with the GP or wider care team where the patient agrees.
