A full Comprehensive Geriatric Assessment in the patient's home: medical history, medication review, cognitive screening, mobility and falls assessment, and a brief review of how the home environment and current supports are working. Dr Wright produces a written report with a clear, agreed management plan; with the patient's consent the report can be shared with the GP or wider care team.

What to expect
From enquiry to written report
An initial enquiry through the public form. Dr Wright's team reviews the enquiry, may gather further information from the patient, family, referrer, GP or insurer, and contacts the patient or family to arrange a convenient visit time. The assessment typically takes 60–90 minutes; a written report follows within a few working days.

Clinical review and suitability
Dr Wright reviews whether the outpatient service is suitable and, where relevant, will try to coordinate any tests or investigations around the appointment so that as much as possible can be addressed in one visit.
Hospital booking and patient information
If an outpatient appointment is suitable, the enquiry is passed to the relevant hospital outpatient booking team.
On the day
On the day, the clinic and hospital teams will coordinate the appointment around the patient's needs where possible.
After the appointment
