Home-visit Comprehensive Geriatric Assessment

Dr Wright offers a private Comprehensive Geriatric Assessment delivered at the patient's home address, across Central, West and North-West London. Same clinical assessment as the outpatient service; same enquiry pathway.

Across Central, West and North-West London

Central, West, and North-West London

The full Comprehensive Geriatric Assessment in the patient's home

A full Comprehensive Geriatric Assessment in the patient's home: medical history, medication review, cognitive screening, mobility and falls assessment, and a brief review of how the home environment and current supports are working. Dr Wright produces a written report with a clear, agreed management plan; with the patient's consent the report can be shared with the GP or wider care team.

When a home visit is more practical than an outpatient clinic appointment

Older adults with frailty, memory concerns, falls, complex medical conditions, or multiple medications — particularly where a home assessment is more practical or clinically appropriate than travelling to an outpatient clinic. Suitable as either a first assessment or a follow-up to a clinic appointment.

From enquiry to written report

An initial enquiry through the public form. Dr Wright's team reviews the enquiry, may gather further information from the patient, family, referrer, GP or insurer, and contacts the patient or family to arrange a convenient visit time. The assessment typically takes 60–90 minutes; a written report follows within a few working days.

Self-pay and insurance

Self-pay fees are published on the Private Healthcare Information Network (PHIN) profile. Insurance recognised — see /fees.html for details.

The safety boundary

Not for emergencies, urgent care, or out-of-hours requests. For emergency help call 999; for urgent but non-emergency advice contact your GP or NHS 111.

How to ask about a home visit

To enquire about a home visit, use the public enquiry form. Dr Wright's team reviews enquiries and will contact you within a few working days.

From enquiry to a clear plan

1

Submit an enquiry

Send a brief enquiry with your contact details, preferred contact method, preferred hospital location if known, and who is referring: GP, consultant, self or insurer.

2

Information gathering

Where needed, Dr Wright's team may gather further information from you, your referrer, your insurer and/or your GP so the enquiry can be considered properly.

3

Clinical review and suitability

Dr Wright reviews whether the outpatient service is suitable and, where relevant, will try to coordinate any tests or investigations around the appointment so that as much as possible can be addressed in one visit.

4

Hospital booking and patient information

If an outpatient appointment is suitable, the enquiry is passed to the relevant hospital outpatient booking team.

5

On the day

On the day, the clinic and hospital teams will coordinate the appointment around the patient's needs where possible.

6

After the appointment

Dr Wright provides a written report and a clear management plan.

What this service does not cover

This is not an emergency service. Dr Wright does not offer urgent care or home visits, and submitting an enquiry does not book an appointment — her team reviews each request and contacts you to discuss next steps. For emergencies call 999; for urgent advice contact your GP or NHS 111.
